  • Adult Standard
  1. Adults - Single Dose Regimen
    1. Give 1500 mg IV once over 30 minutes
  2. Adults - Two Dose Regimen (one week apart)
    1. Dose One: 1000 mg IV over 30 minutes
    2. Dose Two: 500 mg IV over 30 minutes given at one week after first dose

  • Adult Renal (eGFR <30 ml/min)
  1. General
    1. Hemodialysis does not remove Dalbavancin (may be given regardless of Hemodialysis timing)
  2. Adults - Single Dose Regimen
    1. Give 1125 mg IV once over 30 minutes
  3. Adults - Two Dose Regimen (one week apart)
    1. Dose One: 750 mg IV over 30 minutes
    2. Dose Two: 375 mg IV over 30 minutes given at one week after first dose (if not on Hemodialysis)
