Medications
Capsules: 300 mg
Suspension: 125 to 250 mg/5ml
Dosing
Adult (and age >13 years)
Take 300 mg orally twice daily or 600 mg orally once daily
Child (age <13 years old)
Give 14 mg/kg/day divided once to twice daily
Renal Dosing
eGFR <30 ml/min
Adult: Limit to 300 mg/day
Child: Limit to 7 mg/kg/day (max 300 mg/day)
Hemodialysis
: Give renal dose after
Dialysis
every 48 hours
