Framingham Heart Failure Diagnostic Criteria

  • Criteria
  • Major (Heart Failure diagnosis requires 1 or more criteria positive)
  1. Acute Pulmonary Edema
  2. Cardiomegaly
  3. Hepatojugular reflex
  4. Neck vein distention
  5. Paroxysmal nocturnal Dyspnea or Orthopnea
  6. Pulmonary rales
  7. Third Heart Sound (S3 Gallup Rhythm)
  • Criteria
  • Minor (Heart Failure diagnosis requires 2 or more criteria positive)
  1. Ankle edema
  2. Dyspnea on exertion
  3. Hepatomegaly
  4. Nocturnal cough
  5. Pleural Effusion
  6. Tachycardia (Heart Rate >120 beats per minute)
  • Interpretation
  1. Heart Failure diagnosis requires 1 major criteria and 2 minor criteria
  1. Sensitive but not specific (best at ruling out Heart Failure)
  2. Test Sensitivity: 97%
  3. Test Specificity: 79%
