Multidrug Resistance Score, MDR Score, Healthcare Associated Multidrug Resistance Risk in Pneumonia

  • Indications
  1. Identify Pneumonia patients at risk for multi-drug resistance (esp. Healthcare Associated Pneumonia or HCAP)
  • Criteria
  1. Any Antibiotics in the last 90 days
  2. Hospitalization for >2 days in the last 90 days
  3. Poor functional status
    1. Significant debility
    2. Incontinence
    3. Inability to perform Activities of Daily Living (ADLs)
  4. Immunosuppression
    1. Absolute Neutrophil Count <1000
    2. Asplenia
    3. Hematologic Malignancy
    4. Congenital Immunodeficiency
    5. HIV Infection
    6. Prednisone equivalent >10 mg/day for 14 days or more
    7. Other immunosuppressive Medication
  • Interpretation
  1. Score 0-1: Low risk for multi-drug resistant lung infections
  2. Score >=2: High risk for multi-drug resistance lung infections
