Dosing
Adult (or child weight <40 kg)
Give 1 to 2 grams IM or IV every 4 to 6 hours
Use 1.5 to 2 grams IV every 4 hours in
Osteomyelitis
Child (weight <40 kg)
Mild to Moderate Infection
Give 100 to 150 mg/kg/day IM or IV divided every 6 hours
Severe Infection
Give 150 to 200 mg/kg/day IM or IV divided every 4 to 6 hours
Newborns (Weight >2 kg)
Age <1 week
Give 25 to 50 mg/kg IV every 8 hours
Age >1 week
Give 25 to 50 mg/kg IM or IV every 6 hours
Adverse Effects
Interstitial Nephritis
Liver
dysfunction
Occurs with dose >12 g/day
Monitor hepatic panel
