SIADH (off-label)
- Due to its unique Vasopressin inhibition (see mechanism below)
- Limited evidence for use in SIADH

- Semi-synthetic, broad-spectrum NaphthaceneAntibiotic produced by the bacterium Streptomyces aureofaciens
- Inhibits Bacterial Protein synthesis
- Binds to Bacterial 30S ribosomal subunit
- Blocks aminoacyl-tRNA binding to mRNA-ribosome complex
- Other effects
- Inhibits Vasopressin activity at the renal tubules, with secondary diuresis
- Take on an empty Stomach, with large glass of non-dairy fluid

General
- Take 150 mg orally four times daily (or 300 mg orally twice daily)

SIADH
- Take 600 to 1200 mg/day divided four times orally daily
- Effect is delayed 5 to 14 days after dose change
- Avoid modifying dose more than every 4 days
- Food
- Milk and Calcium-rich food decrease absorption
- Take 1 hour before or 2 hours after a meal
- Take with a full glass of water (risk of Pill Esophagitis)
- Pregnancy Category D
