Dosing
Adult
Mild to Moderate Infections
Give 0.5 to 1 g IM or IV every 6 to 8 hours
Severe Infections
Give 1 to 1.5 g IM or IV every 6 hours
Loading Dose protocol
Give 2 g IM or IV, then 1 g every 6 to 8 hours
Renal Dosing
eGFR 35 to 54 ml/min: Dose every 8 hours
eGFR 10 to 35 ml/min: 50% of typical dose every 12 hours
eGFR <10 ml/min: Give first typical dose, then 50% of typical dose every 18 to 24 hours
Dosing
Child (age >1 month)
Mild to Moderate Infections (Typical Dosing)
Give 25 to 50 mg/kg/day divided every 6 to 8 hours
Severe Infections
Give 100 mg/kg/day divided every 6 to 8 hours
Maximum: 2 grams per dose
Renal Dosing
eGFR 40 to 70 ml/min: 60% of typical dose every 12 hours
eGFR 20 to 40 ml/min: 25% of typical dose every 12 hours
eGFR 5 to 20 ml/min: Give first typical dose, then 10% of typical dose every 24 hours
