- Typical Dosing
- Give 1-2 grams IM or IV every 12 hours
- Surgical Prophylaxis
- Give 1 to 2 g IV 30 to 60 min before surgery (or for Cesarean Section, after Umbilical Cord clamped)
- Renal Dosing
- eGFR 10 to 30 ml/min: Give typical dose every 24 hours
- eGFR <10 ml/min: Give typical dose every 48 hours
- Child (Not FDA approved)
- Give 40 to 80 mg/kg/day IV divided every 12 hours
- Pregnancy Category B
- Safe in Lactation
