- Tablets: 100 or 200 mg
- Take with food

- Streptococcal Pharyngitis, Simple Urinary Tract Infection
- Take 100 mg orally twice daily
- Acute Exacerbation of Chronic Bronchitis, Community Acquired Pneumonia, Acute Sinusitis
- Take 200 mg orally twice daily
- Skin Infection
- Take 400 mg orally twice daily

- Give 5 mg/kg/dose orally twice daily
