Dosing
Adult
Oral
Start: 25 mg orally daily
Increase to 50 mg orally twice daily after first day
Increase to 100 mg orally in the AM and 50 to 100 mg orally in PM
Maximum: 200-400 mg divided twice daily
Onset: 30 minutes
Duration: 6 to 8 hours
Intravenous
Infusion concentration: 50 mg/ml
Start: 0.5 to 1 mg/kg IV slowly over 20 to 30 minutes
Maximum: 100 mg/dose
IV Dose in 2023 costs $2500 in U.S.
Onset: 5 minutes
Duration: 2 hours
