  1. Hematoma
    1. Have you ever experienced Bruises or Hematomas >2 cm without Trauma or severe Bruising with minor Trauma?
  2. Hemorrhage
    1. Have you ever had prolonged or unusual bleeding requiring medical evaluation and treatment?
  3. Menorrhagia (women)
    1. Have you ever had prolonged or heavy menstrual periods requiring medical evaluation and management?
  4. Surgery
    1. Have you had excessive bleeding during or after surgery
  5. Tooth Extraction
    1. Have you ever had prolonged after Tooth Extraction that required medical or dental evaluation?
  6. Obstetrics (women)
    1. Have you had prolonged or excessive bleeding following a delivery?
  7. Parents
    1. Is there anyone in your family with a Coagulation Disorder (e.g. Hemophilia, Von Willebrand Disease)
  • Interpretation
  1. Two or more positive answers suggests Bleeding Disorder
    1. Test Sensitivity: 89.5%
    2. Test Specificity: 98.6%
