  1. Relied on others for Activities of Daily Living (e.g. bathing, shopping)?
  2. Prevented from obtaining basic needs (e.g. food, clothing, medical care)?
  3. Verbally abused or threatened?
  4. Forced to sign papers or transfer money?
  5. Emotional, physical or sexual abuse?
  6. Signs of abuse (based on physician assessment) such as withdrawn, malnourished, poor hygiene or Bruises?
  • Interpretation
  1. Any "Yes" answer for questions 2-6 should raise concern
