Teriflunomide, Aubagio

  • Indications
  1. Multiple Sclerosis (relapsing)
    1. Typically in combination with other MS agents (e.g. Interferon Beta, glatiramir)
  • Mechanism
  1. Immunomodulatory agent that inhibits Pyrimidine synthesis
  2. Teriflunomide is an active metabolite of Leflunomide
  • Medications
  1. Teriflunomide (Aubagio) tablets: 7 mg, 14 mg
  • Dosing
  1. Adults: 7 to 14 mg orally once daily
  • Adverse Effects
  1. Hepatotoxicity (black-box warning)
    1. May be severe or fatal, similar to Leflunomide toxicity (uncommon)
    2. Stop Teriflunomide at the first sign of liver toxicity
  2. Gastrointestinal
    1. Diarrhea
    2. Nausea
  3. Neurologic
    1. Paresthesias
  4. Dermatologic
    1. Alopecia
  • Safety
  1. Unknown safety in Lactation
  2. Contraindicated in Pregnancy
    1. Pregnancy Category X in all trimesters
    2. Use effective Contraception
    3. Men and women should not try to conceive until Teriflunomide serum levels are <0.02 mcg/ml
  3. Monitoring
    1. Liver Function Tests (at baseline and then every 6 months)
    2. Complete Blood Count (at baseline and periodically, including with signs infection)
