Dosing
Adults

Give 1.25 g IV every 6 hours administered over 30 minutes

Give 1 g IV every 6 hours administered over 30 minutes

30 to 59 ml/min
Give 0.75 g IV every 6 hours administered over 30 minutes

15 to 29 ml/min
Give 0.5 g IV every 6 hours administered over 30 minutes

<15 ml/min
Avoid unless
Hemodialysis
