  • Indications
  1. Coverage
    1. Gram-Negative Bacteria (aerobes and Anaerobes)
    2. Multidrug Resistant Enterobacteriaceae (Ceftazidime-resistant, ESBL)
    3. Bacteroides
    4. Prevotella
    5. Pseudomonas aeruginosa
  2. Conditions
    1. Complicated intra-abdominal infections (with Metronidazole)
    2. Complicated Urinary Tract Infections including Pyelonephritis
    3. Hospital Acquired Pneumonia (and Ventilator-Associated Pneumonia)
  • Contraindications
  1. Not FDA approved for use in children
  • Dosing
  1. Standard Dose (intra-abdominal infections, Complicated UTI)
    1. Infuse 1.5 g IV every 8 hours
  2. High Dose (Hospital Acquired Pneumonia, Ventilator-Associated Pneumonia)
    1. Infuse 3 g IV over 1 hour every 8 hours
  3. Renal Dosing
    1. eGFR 30 to 50 ml/min: 750 mg IV every 8 hours
    2. eGFR 15 to 29 ml/min: 375 mg IV every 8 hours
    3. eGFR <15 ml/min or Hemodialysis: 750 mg IV Load, then 150 mg IV every 8 hours after Hemodialysis
