  1. Standard dose: 60 mg orally daily
  2. Half dose indications (30 mg orally daily)
    1. Creatinine Clearance 15 to 50 ml/min/1.73m2 OR
    2. Weight <60 kg

  2. Half-Life: 10-14 hours
  3. Onset: 1-2 hours
