Barrel Roll Maneuver, Barbecue Roll Maneuver, Lempert 360 Degree Roll Maneuver

  • Indications
  1. Benign Paroxysmal Positional Vertigo (BPPV) affecting the lateral semicircular canal
    1. Procedure is diagnostic and therapeutic
  • Background
  1. Maneuver maintains lateral semicircular canal perpendicular to the floor while rotating a full 360 degrees
  2. Postulated to move particles from the involved lateral semicircular canal into the utricle
  • Technique
  1. Patient starts by lying supine
  2. Patient's entire body is rolled as a single unit in 90 degree increments for a full 360 degree turn
    1. Roll patient to 90 degrees (on to one side) and stop until Vertigo subsides
    2. Next, roll patient another 90 degrees into prone position and stop until Vertigo subsides
    3. Next, roll patient another 90 degrees on the their other side and stop until Vertigo subsides
    4. Finally, roll patient another 90 degrees until they are once again supine
  3. Numerous variations exist
    1. Starting with rotating head to the opposite side of roll for 15-30 seconds, then rotating head to the other side
    2. Rolling to prone position directly from supine position
    3. Moving to sitting position directly from prone position
